Operation Brown Sugar (OBS) is part of QCHC Chronic Care Model approach to outcome-based health care. The program started in 2003 with a small number of patients and now has grown and spread to include all our sites and all our eligible patients.  OBS is for persons with Type I or Type II Diabetes; the program is designed to educate participants about healthy lifestyles for diabetics and to help them maintain their health.  This program is customized for each individual, but also provides group support meetings. This program focuses on the total health profile of diabetics providing services from each of our service groups: medical, dental and social services.
