Welcome to Vaux Family Health Center. We are here to provide quality primary care services to all children of Vaux Middle School as well as the entire community.

Services will be provided by our primary health care team which consists of a pediatrician, internal medicine physician, health educator, family practice physician assistant, EMT/medical assistant and support staff.

Parents/Guardians of Vaux Students have two options to choose from:
(Both options require written permission from parents for their children who are enrolled in the Vaux Elementary School. Students may receive services without a parent or guardian present. We will attempt, however, to notify parents of any services requested by students.)

Option 1 -Community
Full Benefits Enrollment Program

To be enrolled in this program you must choose for your child, the Vaux Family Health Center as his/her primary care center. This means that your child will receive all of their primary services at this location. Your child will have their own personal provider to assist you with all their health needs.
Other benefits include:

• Convenient hours, including Wednesday late night and 24 hour emergency telephone coverage.
• Comprehensive physical exams
• EPSDT evaluations for children
• Laboratory testing
• Immunizations/TB testing
• Referrals for specialty services
• Health education programs
• Hearing and vision screenings
• Preferred dental appointments at our dental office, located at 25th & Lehigh Avenue
• Nutritional counseling and workshops
• Pyschosocial counseling and workshops
• Referrals to the Vaux Family Care Center for social services

We accept Keystone Mercy Health Plan, Health Partners, Oxford Health Plan, and AmeriChoice (formerly HMA). We also accept Keystone Health Plan East and a number of private insurance plans.

Option 2 - School based service
Limited Student Enrollment

As a student of Vaux Elementary School, your child is entitled to receive limited medical services:

• Immunizations/TB testing
• Evaluation for acute illnesses
• Vaux providers will consult with student's Primary Care Physician if problems are identified
• Emergent/urgent care evaluations
• Vision and Hearing screenings
• Limited lab testing
• Health education and programs for students
• School physical exams

These services are only available during school hours.
