Quality Community Health Care, Inc. is committed to ensuring continued access to care for all patients in accordance with Health Resources and Services Administration (HRSA) Health Center Program requirements.

Please be advised that the following health center locations are temporarily closed to the public as we work toward resuming normal operations:

•Meade Family Health 1600 N. 18th Street Philadelphia, PA 19121

•Cooke Family Health 1300 W. Loudon Street Philadelphia, PA 19141

Because these are in-person clinics, all on-site services at these locations are currently unavailable.

Access to Care During This Temporary Closure

If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room immediately.

For urgent medical needs and continuity of care, patients may receive services at our alternate site:

Quality Community Health Center 2501 W. Lehigh Avenue Philadelphia, PA 19132

Our organization continues to ensure that medical services remain available during this temporary closure.
